Case Management

Coordinated, compassionate support that brings together healthcare providers, service agencies, and families. We help manage care transitions, provide referrals, and ensure ongoing communication—so you and your loved ones feel informed, supported, and never alone in the process.

Available in person only

Individual / Family Counseling

One-on-one emotional support to navigate aging, grief, stress, and life changes—offered with compassion, clarity, and care.

Guidance and support to help families navigate complex dynamics, caregiving roles, and emotional challenges—together, with understanding and strength.

NOTE: If you have traditional Medicare, these services could be covered, free of charge.

**While our specialty is working with seniors, we are also able to offer psychotherapy individual and family counseling to all adults over the age of 18.

Insurance is not accepted at this time, we do hope to offer this opportunity in the future.

Medicaid Application Assistance

Expert guidance through the Medicaid application process, including eligibility, documentation, and renewals—making a complex process more manageable.

Initial Consults

(Part of Case Management / Individual and Family Counseling)

Every journey begins with a thoughtful, in-depth consultation. We’ll review your medical history, daily needs, living environment, and financial considerations to fully understand your situation. From there, we’ll explore support options tailored to your goals—ensuring your needs are met today and into the future.

When indicated in person, clinician will come to your home for the service.
